Kelly Braham, PharmD, BCGP is a board certified geriatric pharmacist. She is the PGY-2 Geriatric
residency program director and serves as a geriatric clinical pharmacist at Upstate University Hospital’s
Community Campus in Syracuse, NY. Her practice sites include the geriatric emergency medicine (GEM)
and a transitional care unit (TCU). She received her doctorate of pharmacy degree from Albany College of
Pharmacy and Health Sciences in 2012. Kelly completed her PGY-1 residency at the Syracuse VA Medical
Center. Kelly is the secretary of the Central New York chapter of New York State Council of
Health-System Pharmacists (NYSCHP) and is a member of the American Society of Consultant
Pharmacists (ASCP), the American Society of Health Systems Pharmacists (ASHP) and the American
Geriatrics Society (AGS). She is on the Consultant Pharmacist editorial review board, the ASCP Pharmacy
Education and Research Council (PERC) and the Medication Safety and Transitions of Care Committee
(TOC). She serves as Preceptor for Albany College of Pharmacy and Health Sciences for APPE students.
As a pharmacist devoted to geriatric care, her focus is centered on addressing the pharmacotherapeutic
management of elderly patients, with an emphasis on ensuring the appropriateness of each drug therapy
regimen and patient education for optimal patient outcomes.
